These Terms & Conditions govern purchases and use of services provided by Rooted Functional Health LLC, doing business as Rooted Functional Medicine & Longevity Lab (“Rooted,” “we,” “us,” or “our”).

By purchasing a program, submitting payment, using this website, or enrolling in Rooted services, you acknowledge that you have read, understand, and agree to these Terms & Conditions.

Program-specific services, pricing, payment arrangements, testing, consultations, and deliverables are described on the applicable checkout page, invoice, order summary, or written offer and are incorporated into these Terms.


1. Nature of Services
 
Rooted Functional Medicine & Longevity Lab provides complementary and alternative educational wellness consulting services.

Services may include educational consultations, nutrition and lifestyle education, review and educational interpretation of client-provided health information and third-party laboratory reports, laboratory-testing facilitation, personalized educational wellness plans, supplement education, digital resources, recorded educational reviews, webinars, and ongoing client support.

Rooted does not provide emergency medical care and does not replace care from an appropriately licensed physician or other healthcare professional.

Dr. Adrienne Guthrie is not a physician licensed by the State of California. She is a board-certified Doctor of Natural Medicine and a Doctor of Functional Medicine, holds two doctoral/PhD-level credentials, and maintains a private professional wellness license through the Professional Wellness Alliance (PWA). These educational and professional credentials do not constitute a California medical license or other California-issued healing-arts license.

Rooted's services are educational, complementary, and wellness-focused in nature. They are not represented as medical diagnosis, medical treatment, prescription of medication, or a cure for disease.


2. No Guarantees

Individual experiences and outcomes vary.

Rooted does not guarantee any specific health outcome, symptom improvement, diagnosis, cure, treatment response, laboratory result, or timeframe for improvement.

Educational recommendations are based on information available at the time of review and may need to be modified as circumstances change.


3. ALL SALES ARE FINAL — NO REFUNDS

ALL PURCHASES ARE FINAL.

Except where applicable law requires otherwise, all payments made to Rooted are non-refundable once payment has been authorized or third-party financing has been funded.

Rooted may begin individualized work shortly after purchase, including:
 

• Client onboarding

• Administrative processing

• Case setup

• Intake review

• Laboratory coordination

• Laboratory ordering

• Scheduling

• Professional preparation

• Educational review

• Reserved professional capacity

• Personalized program preparation
   

These services have value regardless of whether the client ultimately completes every portion of the program.

A refund is not automatically owed because a client:
 

• Changes their mind

• Chooses not to continue

• Does not schedule or attend appointments

• Does not complete required forms

• Does not complete or return laboratory testing

• Submits an insufficient or delayed laboratory specimen

• Chooses not to follow recommendations

• Works with another provider

• Experiences a change in personal circumstances

• Experiences a change in financial circumstances

• Experiences a change in health circumstances

• Develops new symptoms or receives a new diagnosis

• Changes medications

• Becomes pregnant

• Experiences illness, injury, surgery, or hospitalization

• Is dissatisfied with recommendations

• Experiences delays involving independent laboratories or carriers

• Allows a program or laboratory order to expire
   

A change in health or personal circumstances may require services, recommendations, testing, or timelines to be reassessed or modified, but does not automatically create a right to a refund for services already purchased, initiated, performed, or reserved.

Nothing in this policy eliminates any refund or cancellation right that cannot legally be waived.


4. Payment Plans

If you select a payment plan, you are purchasing the entire program at the time of enrollment and choosing to pay the total purchase price in installments.

A payment plan is not a month-to-month subscription unless the written offer expressly states otherwise.

Stopping participation does not cancel the purchase or automatically eliminate remaining payments.

Failure to attend appointments, complete forms, complete laboratory testing, or continue participation does not automatically cancel the remaining agreed-upon balance.

Third-party financing through services such as Affirm, Klarna, Afterpay, or another financing provider is also subject to your separate agreement with that provider.

Rooted is not the lender and does not control financing approval, interest, fees, repayment schedules, or credit reporting.


5. Laboratory Testing

Laboratory testing is performed by independent third-party laboratories.

By authorizing laboratory testing, clients permit Rooted to provide information reasonably necessary to coordinate the authorized laboratory order and receive resulting reports for educational review.

Rooted does not control:
 

• Shipping or carrier delays

• Laboratory processing times

• Sample sufficiency

• Laboratory methodology

• Lost or damaged specimens

• Contaminated or improperly collected specimens

• Recollection requirements

• Laboratory operational delays
   

Laboratory processing and shipping timeframes are estimates and are not guaranteed.

Laboratory reports are educational tools and are not medical diagnoses.

Results requiring medical evaluation, diagnosis, or treatment should be reviewed with an appropriately licensed healthcare professional.


6. Client Responsibility

Clients are responsible for providing accurate and complete information and promptly reporting material changes in health circumstances.

Clients should consult an appropriately licensed healthcare professional before starting, stopping, or changing prescription medications or medical treatment.

Clients are responsible for completing required onboarding documents, laboratory authorizations, appointments, testing procedures, and reasonable program steps.

Failure to complete required participation steps does not automatically cancel the purchase or create a right to a refund.


7. Supplements and Third-Party Products

Supplement information provided by Rooted is educational.

Unless expressly included as part of a purchased package, clients are not required to purchase supplements from a particular vendor.

Products may be manufactured, sold, shipped, or fulfilled by independent third parties and may be subject to those companies' separate return, replacement, shipping, and recall policies.

Rooted is not responsible for acts, omissions, delays, defects, or failures of independent manufacturers, carriers, laboratories, payment processors, financing companies, technology providers, or other third parties.


8. Appointments and Participation

Appointments must be scheduled according to availability and applicable scheduling policies.

Rooted may establish reasonable policies concerning rescheduling, late cancellations, missed appointments, communication response times, program expiration, laboratory expiration, and participation requirements.

Repeated missed appointments, abusive or harassing conduct, nonpayment, material noncooperation, or misuse of Rooted services may result in suspension or termination of services.

Termination does not automatically eliminate financial obligations already incurred.


9. Service Concerns

If you believe Rooted has materially failed to provide a purchased service, you agree to contact us in writing and provide a reasonable opportunity for the concern to be reviewed.

When appropriate, Rooted may address a concern through a corrected deliverable, rescheduled service, replacement consultation, omitted service, or comparable remedy.

A discretionary accommodation, credit, extension, or courtesy does not modify these Terms or create an obligation to provide the same accommodation in other circumstances.


10. Chargebacks and Payment Disputes

Before initiating a chargeback or payment dispute regarding an authorized purchase, you agree to contact Rooted in writing and provide a reasonable opportunity for the matter to be reviewed.

Initiating a chargeback or payment dispute does not automatically cancel a valid purchase or eliminate amounts legitimately owed.

If a payment dispute occurs, Rooted may provide relevant documentation to the financial institution, card issuer, financing provider, payment processor, collection service, arbitrator, court, or other reviewing party.

Documentation may include:
 

• Checkout records

• Signed agreements

• Electronic acknowledgments

• Payment authorization

• Invoices

• Communications

• Scheduling records

• Laboratory orders

• Shipping or tracking records

• Portal activity

• Educational materials

• Protocols

• Recorded reviews

• Consultation records

• Records demonstrating services initiated or performed
   

Amounts that remain legitimately due may be pursued through lawful collection remedies.


11. Intellectual Property

All Rooted protocols, plans, videos, webinars, templates, educational materials, documents, presentations, and proprietary resources remain the intellectual property of Rooted Functional Health LLC.

Purchasers receive a limited, personal, noncommercial, nontransferable right to use purchased materials for their own education.

Materials may not be copied, published, sold, distributed, commercially exploited, or publicly shared without written permission.


12. Privacy

Rooted may collect and use information reasonably necessary to provide services, administer accounts, process payments, coordinate laboratory testing, communicate with clients, and comply with applicable legal obligations.

Additional information regarding collection and use of information may be provided in Rooted's Privacy Policy.


13. Limitation of Liability

To the fullest extent permitted by applicable law, Rooted is not responsible for indirect, incidental, special, consequential, or punitive damages arising from participation in its educational wellness services.

Rooted is not responsible for acts or omissions of independent laboratories, carriers, manufacturers, financing companies, payment processors, technology providers, or other third parties.

Nothing in these Terms excludes liability that cannot legally be excluded.


14. Dispute Resolution

The parties agree to first attempt in good faith to resolve disputes through written communication.

Except where prohibited by law or where a matter appropriately belongs in small claims court, disputes arising from these Terms or Rooted's services will be resolved through binding arbitration through a mutually agreed provider under applicable consumer arbitration rules.

California law governs these Terms except where another applicable law controls.

If any provision is found unenforceable, the remaining provisions remain in effect.


15. Changes to These Terms

Rooted may update these Terms from time to time.

The Terms applicable to a specific purchase are those in effect when that purchase is completed unless the client subsequently agrees to updated terms.
